Music Love Academy occasionally photographs or records lessons, rehearsals, classes, recitals, and special events for
educational, promotional, and marketing purposes. These materials may be used on the academy’s website, social
media platforms, printed materials, and other promotional outlets.
By submitting this form, I request that my child (or myself) not be photographed or recorded for promotional or public
use.
I understand and acknowledge the following:
• Reasonable efforts will be made to exclude my child (or myself) from photos and videos used for promotional purposes.
• This opt-out does not apply to security camera footage, which is used strictly for safety and monitoring purposes.
• In group settings, complete exclusion from incidental background appearances cannot be guaranteed.
• This waiver remains in effect unless revoked in writing.
